A customer support ticket form with cascading dropdowns and conditional escalation.

Features demonstrated:

  • 3-level cascading dropdowns: product selection drives category options, category drives sub-category options
  • Severity "Outage" auto-sets priority to "Critical" via a setValue rule effect
  • Dynamic description label changes to request outage-specific details when severity is "Outage"
  • Escalation contact field appears only when both "blocking" AND "company-wide" toggles are enabled (and condition)
  • Autocomplete field for searching related tickets
